7. Indemnity Statement
I hereby give permission for the above-named participant to participate in the FCBarcelona Camp Singapore 2012 to be held from 22nd October to 26th October 2012. I acknowledge that, in the event that he/she requires medical attention during the FCBarcelona Camp Singapore 2012, then I agree to SPORTING JUNIORS arranging for any and all necessary and appropriate medical treatment. While reasonable precautions will be taken, I will not hold SPORTING JUNIORS, its Management, Staff, Coaches and invited Coaches, liable for any death, disability, permanent injury, loss of property or any loss however arising from any cause whatsoever at anytime during his/her participation in all activities related to the programme. I acknowledge that SPORTING JUNIORS reserves the right to remove from the FCBarcelona Camp Singapore 2012, without refund, any participant who is found bullying other participants or is otherwise behaving in a way which disrupts the camp activities.

Please crossed and make payable to "Sporting Juniors" and mail to "74A Keng Lee Road, #04-01, Singapore 219250". - You will need to write your Child's Name, IC Number, Contact Number to help us locate in our spreadsheet. Indicate latest Jersey Size with the attachment provided. All this written on the back of the cheque.

Please come down to our office at Kong Beng Industrial Building, 41 Jalan Pemimpin #05-01, Singapore 577186., Weekdays only, Monday to Friday, from 10.00am to 1.00pm. (Please inform us before you come).
Wire Transfer For those living overseas, we would provide you with the Bank Wire Transfer details once we receive your request to make payment.
* Your child’s participation will only be confirmed once payment is made. A receipt will be issued to you.
* Registration fees may be refunded within 14 days of receipt of the duly completed registration forms, subject to an administrative charge of S$50.00. Thereafter, no registration fees will be refunded
First Come First Serve Basis - We go by the policy of First Come First Serve Basis. If the numbers are filled up for the afternoons and your child doesn't have a place, we will return your cheque unless you indicate that you would like us to hold it for the mornings should there be any place available later.
